Extended Occupational Health Assessment

An extended assessment is a more comprehensive and in-depth assessment of a patient’s problem by one of our Chartered and State Registered Physiotherapists.

The assessments include more objective measures, a more comprehensive report (including fitness for normal, and modified duties) with insights into the patient’s current status, future status and treatment pathways.

Comprehensive assessment

  • Extended assessment (provides a status report and advice on potential treatment pathways)
  • Ask specific questions (e.g. patient has been off work for 20 weeks is it likely they will ever return on normal duties)
  • Longstanding musculoskeletal problems assessed
  • Ability to work (including sickness absence)
  • Modifications of normal duties

Patients who have extended assessments are scrutinised via our clinical scrutiny department:

Senior clinicians

Every report is read and checked by one of our senior clinicians to assess suitability and appropriateness. Any discrepancies and/or questions are then asked back to the assessing therapist for comment/clarification. Jargon/abbreviations/medical terms are explained.

Expert advice

Our scrutiny therapists offer you advice on how to proceed (with the management of the patient). Note, this is advice only the final decision is always yours.

Robust clinical governance and reporting

Each Extended assessment generates a report including:

  • pain levels
  • subjective markers
  • restriction of movement (which ones and how much)
  • objective markers
  • ability to perform normal work role (including predicted absence times)
  • advice on modification of duties
  • permanent changes to normal role
  • diagnosis
  • prognosis
  • recommendations for further tests/medical opinions/treatments

Suitable for

Suitability

  • All patients (especially suited for long term and chronic problems)
  • All musculoskeletal conditions (one body area per referral e.g. neck/shoulder pain is acceptable, but neck/ankle pain would require two referrals e.g. one for the neck and one for the ankle)
  • Long term sickness
  • Recurring problems
  • Patients with red flags/high yellow flag scores
  • Patients who did not respond to other treatments
  • After surgery status

Not for

  • Multiple body areas (a separate referral is needed per area)
  • Anyone under the care of another Physiotherapist/Osteopathist/Chiropractor/Consultant
  • Medico legal purposes

Report Suitable for

Suitability

  • Patient status (all subjective and objective factors)
  • Long term management of the patient
  • Physiotherapist opinion on overall and future work capability (sickness absence)
  • Modification of duties (including permanent changes)
  • Suggestions for workplace adjustments and/or assessments
  • Diagnosis and prognosis with time frame for expected progress
  • Future management of condition

NOT for

  • Medico legal cases
  • Work place adaptions (an actual onsite review required)
  • Multiple body areas (e.g. ankle and neck)

Referral Guidance

  • The patient must have a muscular skeletal problem centred around a specific area of the body (e.g. back, neck, leg etc.).
  • Please state any questions that you would like answered e.g. can the patient kneel after their knee replacement (questions asked after the fact can be impossible to answer)
  • A job role and description of the ‘real world’ requirements/needs of the service is beneficial otherwise the Physiotherapist will rely on the patient’s description of their duties. Please be as specific as possible and not use a generic job description as found from the patients contract e.g. ‘lifting as required’ should be specific – lifting 40kg washing machines into homes then push/pull to install them as a 2-person team.
  • Include any other relevant medical information that is known
